Client Document Requirements
Please review this list of required information and documents carefully so that you may help your client make sure all the correct documents are on file prior to your clients’ event:
Questionnaires
We will send out two questionnaires for your clients to complete. You may also complete them on your clients’ behalf:
Vendor Information Questionnaire
Due 4 weeks prior to the event
Informs us of all vendors coming onto the property
Helps us determine if we already have vendor COIs on file
Final Details Questionnaire
Due 2 weeks prior to the event
Informs us of all the details of the event.
Client Documents
We need the following three client documents no later than 4 weeks prior to your clients’ event:
Certificate of Event Insurance

Fee Acknowledgement Form
Required for any contract signed prior to 12/1/19.
Clients can sign this document online here.
COVID Release of Liability
Due no sooner than 7 days prior

Vendor Documents
The following is a list of documents we are required to have on file from your clients’ vendors, no later than 4 weeks prior:
COI for Tent, Rental, and Catering
If your client is using anyone on our Vendor Guide, we already have this on file.
ABC License for Bartenders
If alcohol is served in any form, it must be served by an ABC licensed bartender.
Planner Agreement Form
We require every planner to sign this for every event.
